| PATIENT INFORMATION Insurance Montclair Physical Therapy, Inc. is endorsed by hundreds of Bay Area physicians and is a Preferred Provider for the Physical Therapy Provider Network (PTPN), which includes more than 1,000 major insurance companies. We accept private insurance, Worker's Compensation, and Medicare. In keeping with our commitment to better serve each patient, please call our office so we can verify acceptance of your insurance plan. You may also visit the PTPN website to view a list of the plans we currently accept. Please note that most health insurance plans include coverage for Physical Therapy, but do not cover Massage Therapy, Acupuncture, Personal Training, or Pilates. New Patient Forms As a patient at our clinic, you may be requested to complete one or more of the following forms. In an effort to save time when you come in, you can print the form(s) directly from this site. Just click on the link below to open the form you need, then use the File > Print function on your computer to print the form. (You will need to have Adobe Reader on your computer in order to open and print the forms. We recommend downloading only the most recent version of Adobe Reader; the Adobe Reader download is free.) at 510-339-2116. Cancellation Policy There is no charge for appointments canceled at least 24 hours in advance. In order to be fair to others who need appointments, we charge the full fee for cancellations made less than 24 hours in advance, or for no shows. This policy applies to Physical Therapy, Massage Therapy, Personal Training, Pilates, and Acupuncture appointments.
